Who We Are
The LiveWell Collective, LLC is a group therapy practice providing telehealth services to clients located in Florida. You can reach us at connect@thelivewellcollective.org or (712) 524-5433.
Information You Choose to Share
Our website includes a contact form, a webinar request form, a careers form and a newsletter sign-up. These forms collect what you enter: your name, email address, phone number, the choices you tick, and your message.
That information is sent securely to our practice. We use it to respond to you, and for newsletter sign-ups, to send the updates you asked for. Service providers that help us operate the website and manage inquiries process this information for us.
Please do not include detailed health information in a website form. The first consultation call is the right place for that.
Cookies, Analytics and Advertising
This site uses:
- Google Analytics, to understand how the site is used
- Google Ads, to measure whether our ads lead to inquiries
- the Meta (Facebook) Pixel, for advertising measurement
These tools may set cookies and receive technical information such as pages visited, device and browser type, approximate location and referring page, including whether a visitor reached our thank-you page after sending an inquiry. We do not send the contents of any form to these tools.
You can opt out of some of this measurement:
- through Google's Ad Settings
- with the Google Analytics opt-out browser add-on
- through Meta's ad preferences
- through your browser's cookie controls
How We Use and Share Information
We use the information described above only to respond to inquiries, provide the information you requested, send newsletters to people who signed up, operate and improve the site, and measure our advertising.
We do not sell personal information. We may disclose information if required by law.
Your Clinical Records
Therapy services and clinical records are handled separately from this website. They are protected under HIPAA and described in the Notice of Privacy Practices provided when services begin.
Using the website or its forms does not create a therapist-client relationship.

You can unsubscribe at any time using the link in any email, or by emailing connect@thelivewellcollective.org.
Children's Privacy
This website is not directed to children under 13. Parents or guardians reach out on a child's behalf.
Security
We use reasonable safeguards to protect the information described on this page. No method of transmission over the internet is completely secure.
Changes to This Policy
Updates will be posted on this page with a new effective date.
