Contamination OCD

Contamination OCD goes far beyond liking things clean. It can make everyday life feel dangerous, with constant worry about germs, illness or things that feel dirty or wrong. Washing and avoiding bring quick relief, but the fear keeps coming back stronger.

With the right approach, people with contamination OCD can touch, share and live more freely again.

What it can look like

  • Fear of germs, dirt, bodily fluids, chemicals or getting sick
  • Feeling that certain people, places or objects are contaminated
  • Long or repeated handwashing, showering or cleaning
  • Avoiding doorknobs, public restrooms, school supplies or shared food
  • Asking others to wash, change clothes or confirm something is clean

When the whole family is affected

Contamination fears often spread to everyone in the home. Family members may be asked to change clothes at the door, wash their hands a certain way, or keep some rooms off limits. These requests usually come from real distress, and families go along with them out of love.

Over time, though, these rules can shrink the family's life and keep OCD strong. We help families understand this cycle without blame, and we plan together how to step back from accommodation gradually and kindly. For children and teens, parents learn what to say in the moment and how to encourage brave choices. Adults can involve a partner or family member in sessions if that feels helpful.

How the cycle keeps going

Each time washing or avoidance lowers anxiety, the brain learns that the danger was real and that the ritual was necessary. Over time, more things start to feel contaminated and the rules grow stricter. Reassurance from others works the same way.

Family members often end up washing, cleaning or avoiding too, simply out of love. We help families gently change these patterns together.

How we help

We use Exposure and Response Prevention (ERP), the gold standard treatment for OCD. Together we build a gradual exposure plan, starting with situations that feel challenging but manageable, such as touching a doorknob and waiting before washing.

Step by step, exposures move up the ladder while you practice resisting washing, cleaning and checking. Exposures happen in session and as between-session practice at home and in daily life. We also work on reducing reassurance seeking, and for children and teens, we coach parents on stepping back from accommodation.
