Harm OCD
Harm OCD involves unwanted thoughts or images about harm coming to someone, or about causing harm yourself. These thoughts can feel terrifying and deeply out of character. Many people keep them secret for years, afraid of what the thoughts might mean.
Here is what we want you to know: these thoughts are a common OCD theme, and they are treatable.
Why the thoughts feel so distressing
People with harm OCD are usually caring, gentle and conscientious. The thoughts are upsetting precisely because they go against the person's values. OCD tends to latch onto what matters most to us, so a loving parent may fear harming their child, and a kind teen may fear hurting a friend.
Having a thought is not the same as wanting it or acting on it. The distress you feel is a sign of how much you care.
You are not alone
Harm thoughts are one of the most common OCD themes, and also one of the least talked about. Because the thoughts feel so shameful, many people search online for answers late at night, or quietly avoid the people they love most. This can lead to isolation and exhaustion.
Speaking with someone who understands OCD can bring real relief. In our work, nothing you share will shock us, and we will help you understand why your brain is responding the way it is. Parents of teens are welcome to reach out first if their child is struggling but not yet ready to talk.
What it can look like
- Checking that doors are locked, stoves are off or others are safe
- Avoiding knives, driving or being alone with loved ones
- Mentally reviewing past moments to make sure nothing bad happened
- Confessing thoughts or asking others for reassurance
- Trying to push thoughts away or replace them with good ones
How we help
We use Exposure and Response Prevention (ERP), the gold standard treatment for OCD. A well-trained ERP therapist does not shy away from scary themes. We approach them calmly, without judgment, and at a pace you agree to.
Treatment focuses on changing your relationship with the thoughts rather than arguing with them. Exposures might include being near a kitchen knife while cooking, or spending time with a loved one without checking. Over time you practice resisting checking, avoidance, confessing and mental reviewing, and the thoughts lose their grip.
